We are committed to doing our part by offering free SAGE-SR screen and assessment to healthcare workers across America as a way of saying thank you.

SAGE-SR SCREENING MODULE
The SAGE-SR (Self-Report) screen asks a series of qualifying questions to identify potential mental health conditions requiring further evaluation.
When potential mental health conditions are indicated, the SAGE-SR assessment module is initiated.
COMPREHENSIVE ASSESSMENT
MODULE
51% of individuals diagnosed with depression have a co-occurring mental condition such as anxiety. However, brief screens, like the PHQ-9 depression screen, are limited to screening for just one mental health condition.
Powered by A.I. and machine learning, the SAGE-SR addresses 30 of the most common mental health conditions in a single 15-minute assessment session.
REVIEW YOUR SAGE REPORT WITH YOUR PHYSICIAN
Your SAGE-SR report lists all potential primary and co-occurring mental health conditions. Conditions with minimal or absent symptoms are also indicated.
If you choose, you can share your report with your healthcare provider to confirm any diagnoses.
